Position Summary:The Field Care Navigator will be responsible for assisting with care coordination and complex caremanagement duties. The role develops and maintains the care management of the member both in the home and telephonically, working to improve member outcomes.Responsibilities:Assess members in the home or other face-to-face setting to assess the members wellbeing and develop an individual plan of careEvaluate the home environment for safety, infection control, and community resource needsAct as first level of support for the member to resolve patient related issues to ensure a positive member experienceIdentify opportunities to manage the member-centric care plan, problems, barriers, interventions, and goals as appropriatePartner with the Healthmap Interdisciplinary Care Team to collaborate on opportunities to improve members health outcomesServe as a resource to members and physicians while acting as a liaison between community resources, programs, and other relevant servicesEducate Healthmap members on kidney health, related co-morbid conditions, and renal replacement therapyDrive and shape clinical strategy to improve outcomes for members with chronic kidney diseaseDocument and maintain accurate member records that will identify all patient communication and plan. Maintain thorough documentation of all provider meetings/interactions and member interactions for consistency and coordination and compliance with regulatory standards Ensure company adherence with legal and regulatory requirementsAssist as a preceptor, mentor, and coach for new hire orienteesDemonstrate HIPAA understanding and complete confidentiality protocolsFollow all company-defined security/operational policies and proceduresUse multiple technical applications including Microsoft Word, Excel spreadsheets, electronic calendars, email, timekeeping and expense applications, and other software applicationsPerform other duties as assignedRequirements:Masters degree in nursing requiredActive, unrestricted RN license required; nurse practitioner or advanced practice nurse required in assigned state (WA)3 years of experience in care management rolesExperience in a nephrology practice, dialysis center, home health delivery or transplant center preferredPrior experience building and managing relationships with health care providers or patients preferredExperience in customer service strongly preferredExperience with Medicare and Medicaid preferredProof of valid and unrestricted drivers license required. This position requires travel within assigned region to visit patients in their homesAssigned state residency required (WA)Skills:Excellent verbal, written, listening and presentation skillsAbility to show compassion and empathy for patientsDemonstrated interpersonal relationship skillsAbility to multitask, prioritize, and create solutions in a fast-paced environmentStrong critical thinking and analytical skillsMust be proficient in Microsoft Office: Outlook, Word, Excel, PowerPointTravel:Travel over 50%, primarily in the Seattle/Tacoma, WA areasCompensation range: $99,000 - $135,000 (dependent on specific market/region as well as experience of the candidate selected) with a yearly bonus potential.Benefits: Competitive: Paid Time Off, Medical, Dental, Vision, Short Term/Long Term Disability, 401K with match and other voluntary benefits as elected.#LI-Hybridby Jobble
