Your responsibilities will include conducting assessments diagnosing and treating health conditions and developing comprehensive care plans to ensure optimal patient outcomes.Key Responsibilities:

• Perform thorough physical examinations and health assessments to evaluate patient needs and conditions.
• Diagnose and manage acute and chronic illnesses developing treatment plans that include medication management and lifestyle recommendations.
• Collaborate with healthcare teams including physicians and specialists to ensure cohesive patient care and followup.
• Educate patients and their families on health management treatment options and preventive care strategies to promote wellness.
• Maintain accurate and timely records of patient encounters treatment plans and outcomes in electronic health records (EHR).
• Stay informed about the latest advancements in healthcare practices and contribute to quality improvement initiatives within the organization.
• Serve as a patient advocate providing support and guidance throughout the healthcare process while respecting patient autonomy and confidentiality.Requirements
  • Master s degree in Nursing (MSN) from an accredited program.
  • Valid state NP license and current CPR certification.
  • Previous experience as a Nurse Practitioner preferred; specialized training in areas such as family practice pediatrics or geriatrics is a plus.
  • Strong clinical skills critical thinking abilities and excellent interpersonal communication skills.
