Care Manager MSW - Everett Medical Center
SCHEDULE: Monday-Friday, 8:00 AM- 4:30 PM
Remote role based at PRMCE Colby with local travel to see clients.
This is a temporary position funded through 6/30/25
The enhanced care management (ECM) program facilitates client
integration, improves quality of care, and promotes a safe home
environment. This state-funded pilot program supports clients
through increased access to a case manager, access to education and
resources, and frequency of home visits. Medicaid Fee-for-Service,
Managed Medicaid, and dual Medicare-Medicaid eligible adults who
are medically ready to transition from acute care but are unable to
due to transition barriers are eligible for enrollment in this
service.
